元祐七年二月某日，龍圖閣學士左朝奉郎知潁州蘇軾狀奏。臣伏見本州潁上縣白馬村，有梵僧佛陀波利真身塔院舍，約四五十間，元無敕額。父老相傳佛陀波利本西域僧，唐儀鳳中遊五臺，禮文殊師利，見老人，令復還西域，取佛頂尊勝陀羅尼經。佛陀波利用其言，往返數萬里，以永淳中取經而還，至今流布，而佛陀波利於潁上亡沒，里俗相與漆塑其身，造塔供養，時有光景，頗著靈驗，不敢具述。臣於諸處見唐人所立《尊勝石幢刊記》本末，與所聞父老之言頗合。今年正月，大雪過度，農民凍餒無所，祈禱境內諸廟未應。聞父老以佛陀波利為言，臣即遣人賫香禱請，登時開霽，人情翕然歸向，詣臣陳狀，願乞敷奏，乞一敕額，庶幾永遠不致廢壞。須至奏乞者。
