君諱仲夔，字彥之。曾祖諱元佐，是為魏恭憲王。祖諱允言，贈安遠軍節度使，同中書門下平章事，封密國公。父宗悅，前左屯衛大將軍、池州團練使，祁國公。君官至右千牛衛將軍，坐法廢。
熙寧元年，年二十二，以五月二十五日卒。至某年某月某日，葬河南府永安縣。妻郭氏，有六男子，死者四人。士㱺今為右監門率府率，一人尚幼。銘曰：
托靈皇宗慶之多，終以無祿傷如何，棄
