元豐四年正月二十一日，予將往岐亭。
宿於團封，夢一僧破面流血，若有所訴。
明日至岐亭，過一廟，中有阿羅漢像，左龍右虎，儀制甚古，而面為人所壞，顧之惘然，庶幾疇昔所見乎！遂載以歸，完新而龕之，設於安國寺。
四月八日，先妣武陽君忌日，飯僧於寺，乃記之。
責授黃州團練副使眉山蘇軾記。
